A sample notice of claim to adverse party is set forth below:

Dear [Defendant]:

This law office has been retained to represent [Name of Plaintiff] to prosecute a claim against you for personal injuries arising out of the automobile accident that occurred on [Date] at [Location].

On behalf of our client, I request that you provide me with the name of your liability insurance carrier and your policy number.
