A sample notice of claim to adverse party is set forth below:

Dear [Defendant]:
This law office has been retained to represent [Name of Plaintiff] to prosecute a claim against you for personal injuries arising out of the automobile accident that occurred on [Date] at [Location].
On behalf of our client, I request that you provide me with the name of your liability insurance carrier and your policy number.
Neither my client nor this firm has notice that your are represented by an attorney in regard to this incident. If that is not the case, and you in fact are represented by legal counsel, please forward this letter to your attorney for an immediate reply.
In any event, I suggest that you contact your insurance carrier and/or an attorney as soon as possible regarding my client’s claim against you.
