Dear [Insurance Carrier]:
This law office has been retained to represent [Name of Plaintiff] to prosecute his/her claim against you insured [Name of Defendant] for personal injuries arising out of an automobile accident that occurred on [Date] at [Location].
Please provide this office with a statement of [Name of Defendant]’s policy limits, the identity and nature of coverage provisions, and whether said policy was in force and effect at the time of the above-mentioned accident. If deemed necessary, please obtain your insured’s written consent to furnishing this information.
